The Guerneville Dentist (“we,” “us,” or “our”) operates the website at theguernevilledentist.com (the “Website”). This Website Privacy Policy explains how we collect, use, and disclose information from visitors to this Website, and describes the rights available to California residents under the California Consumer Privacy Act of 2018 (“CCPA”), as amended by the California Privacy Rights Act (“CPRA”).

This policy governs this Website only. If you are a patient, our handling of your Protected Health Information (“PHI”) is governed by our Notice of Privacy Practices under HIPAA, not this Website Privacy Policy.

1. Information We Collect

We collect the following categories of Personal Information as defined by the CCPA/CPRA:

  • Identifiers — name, email address, phone number (when you submit our contact form).
  • Customer records / contact information — information you voluntarily provide, such as questions or requests contained in the contact form’s message field.
  • Internet or other electronic network activity — pages viewed, referring URL, approximate location derived from IP address, device/browser characteristics, interaction events, and session timing, collected via cookies and similar technologies through Google Analytics (loaded via Google Tag Manager).
  • Technical server data — your IP address, user agent, and request metadata collected automatically in standard web server logs for security and diagnostics.

Sensitive Personal Information: We do not intentionally collect sensitive categories of Personal Information (such as government identifiers, precise geolocation, account credentials, racial or ethnic origin, religious beliefs, genetic data, biometric data, health or sexual orientation data) through this Website. PHI is handled separately under HIPAA.

2. Sources of Information

We receive information (a) directly from you when you submit the contact form, email us, or call our office; (b) automatically from your browser when you visit the Website; and (c) from third-party service providers such as Google.

3. How We Use Information

We use the information we collect only for the following business purposes:

  • To respond to inquiries you submit through the contact form.
  • To schedule, confirm, or follow up on appointments you request.
  • To operate, maintain, secure, and improve the Website.
  • To detect, prevent, and respond to security incidents, fraudulent or illegal activity, and to protect our rights.
  • To comply with legal obligations.
  • To measure aggregate Website usage and improve our content.

4. How We Share Information

We disclose Personal Information only to the following categories of recipients, and only for the purposes described above:

  • Service providers / processors that operate the Website on our behalf, including our web hosting provider, our form processor (Gravity Forms), Google Tag Manager and Google Analytics (for aggregate usage measurement), Google reCAPTCHA (for spam and abuse prevention), and Google Search Console (for search performance diagnostics).
  • Legal, regulatory, or public authorities where required by law, court order, subpoena, or to protect our legal rights.
  • Successors in the event of a merger, acquisition, financing, reorganization, bankruptcy, or sale of assets, subject to equivalent protection of the information.

5. We Do Not Sell or Share Personal Information

We do not sell Personal Information, and we do not share Personal Information for cross-context behavioral advertising, as those terms are defined under the CCPA/CPRA. We have not done so in the preceding 12 months. We do not knowingly sell or share the Personal Information of consumers under 16 years of age.

6. Cookies and Similar Technologies

The Website uses the following cookies and similar technologies:

  • Strictly necessary — WordPress and our caching system use short-lived functional cookies when you interact with forms or if you log into the administrative area.
  • Analytics — Google Analytics (loaded via Google Tag Manager) uses cookies and similar identifiers to measure aggregate site usage.
  • Security — Google reCAPTCHA uses cookies and similar technologies to distinguish humans from automated traffic on the contact form.

7. Global Privacy Control (GPC)

We recognize the Global Privacy Control signal. Because we do not sell or share Personal Information for cross-context behavioral advertising, no opt-out is required. If we ever begin such sharing, we will treat a valid GPC signal as a request to opt out of sale/sharing with respect to that browser or device.

8. Data Retention

We retain contact-form submissions and related correspondence for no longer than reasonably necessary to respond to your inquiry and to document our operations, typically not more than three (3) years, unless a longer retention period is required by law. Server log data is retained for up to ninety (90) days. Aggregated analytics data may be retained indefinitely in non-identifiable form.

9. Security

We use industry-standard administrative, technical, and physical safeguards to protect Personal Information. No method of internet transmission or electronic storage is 100% secure, and we cannot guarantee absolute security.

10. Your California Privacy Rights

If you are a California resident, you have the following rights with respect to Personal Information that this Website has collected about you:

  • Right to Know / Access — the categories and specific pieces of Personal Information we have collected, the categories of sources, the business purposes for collecting it, and the categories of third parties with whom we have disclosed it.

How to Exercise Your Rights

To submit a verifiable consumer request, contact us using any of the methods below. We will respond to verifiable requests within the time frames required by law (generally 45 days, extendable by an additional 45 days when reasonably necessary, with notice to you).

We will need to verify your identity before fulfilling your request, typically by matching information you provide with information already in our records. We may decline or limit a request where verification is not possible or where an exception under the CCPA/CPRA applies.

Authorized Agents

You may designate an authorized agent to make a request on your behalf. We may require the agent to provide written authorization signed by you and may separately verify your identity. We may deny a request from an agent that does not submit proof of authorization.

Shine the Light (California Civil Code § 1798.83)

California residents may request information about disclosures we have made of Personal Information to third parties for those parties’ direct marketing purposes. We do not disclose Personal Information to third parties for their direct marketing purposes.

11. Children’s Privacy

This Website is not directed to children under 13, and we do not knowingly collect Personal Information from children under 13. If you believe a child under 13 has provided Personal Information to us, please contact us and we will delete it.

12. “Do Not Track”

This Website does not respond to browser “Do Not Track” signals because there is no common industry standard for interpreting them. See the Global Privacy Control section above for the signal we do honor.
